The Lewes and District Palliative Care and Support at Home Project - Project

Project overview

The aim of this existing project is to improve the quality of life of terminally ill patients, and to support patients' families. The project will enable more people to receive palliative care in their own homes - a clinical nurse specialist will visit patients on referral, arranging the appropriate medication, physiotherapy, complementary therapy, counselling and practical aids. With this support, the patient is less likely to be admitted to hospital and will have the choice to die at home or in a hospice.
