Children's Hospice Association Scotland's (CHAS) five year 'Transitions Team' project seeks to assist young people with life limiting conditions into adulthood and adult services. The initiative will allow the group to employ staff to work alongside relevant statutory and voluntary agencies to deliver a holistic support package that ascertains the needs, concerns, goals and aspirations of the young person and their family through 1:1 support, which can last between one to three years. The project will support 65 young people and their families to make decisions about their care whilst taking into consideration their complex conditions as they move into adult services. This grant will fund one FT transitions manager, two FT transitions workers, professional memberships, travel expenses, recruitment costs and PC's.
